The Forms Landscape Is Fragmented

South Carolina plumber license applications don't use a single unified form. You're dealing with three separate entities — the Contractor's Licensing Board (CLB), the Residential Builders Commission (RBC), and the CAGC SC Trades Certification Program — each with their own forms, portals, and payment requirements.

Which forms you need depends entirely on which license track you're pursuing. Getting the wrong forms wastes weeks. Submitting forms to the wrong board wastes more. Here's exactly what each track requires.

Commercial Mechanical Contractor (CLB) Forms

The Contractor's Licensing Board manages all forms through the LLR website at llr.sc.gov/clb/clb_apps.aspx. These are the forms you need for a commercial plumbing (PB) license:

Doc 165 — General & Mechanical Contractor Application: The main application form for the business entity. This is where you declare the license classification (Plumbing/PB), your target license group (1-5), and provide the business entity details. You cannot submit this until after you've passed both PSI exams — the CLB operates on a "test first, apply second" sequence.

Doc 168 — PQP/QP Initial Application: The Primary Qualifying Party certification form. Filed simultaneously with Doc 165, this designates the individual who holds the trade credentials and will be personally responsible for supervising all plumbing work under the license. The QP must attach their exam pass sheets and meet the experience requirements.

Doc 167 — Work Experience Affidavit: Each affidavit documents the QP's commercial plumbing experience under a specific supervising contractor. The form must be signed by the supervising contractor and notarized — not just signed, but notarized. You need enough affidavits to verify at least two years of commercial experience within the past five years.

Doc 172 — Owner-Prepared Financial Statement: A balance sheet template for Group 1 and Group 2 applicants who don't have CPA-prepared financials. You fill in your business assets, liabilities, and net worth. Groups 3 and 4 require a financial statement compiled by a licensed CPA; Group 5 requires a full balance sheet audited by a licensed CPA.

Doc 181 — PQP/QP Revision & Transfer Form: Not needed for initial applications, but critical to have on hand. This is the form you file when your qualifying party changes — whether they leave the company, retire, or you need to transfer the QP certification to a new business entity.

The CLB application fee is $350, submitted with the Doc 165 package.

Residential Specialty Plumbing (RBC) Forms

The RBC has moved most of its application process online through the portal at llr.sc.gov/res/resplumber.aspx. The key requirement:

RBC Online Application: The RBC application collects your personal information, experience verification, and supporting documents in one submission. You'll upload your SLED background check, credit report (dated within 30 days), and experience affidavits directly through the portal.

The RBC application fee is $135, paid at submission. After exam passage, the license activation fee is $220.

The critical payment rule: For paper applications to the RBC, payment must be by certified check or money order. Personal checks and company checks result in immediate application rejection — not a processing delay, but a complete return of your entire packet. This is one of the most common avoidable mistakes applicants make, and it's particularly frustrating because the board doesn't flag it as an error — they return everything without processing and you start over.

CAGC Trade Certification Forms

Individual Journeyman and Master Plumber trade certifications go through the CAGC Foundation at cagc.org/sctradescert:

CAGC Application: The standard application for trade certification. You provide documentation of your on-the-job training hours (2 years for Journeyman, 4 years for Master with at least 2 at the Journeyman level) and pay the $35 processing fee. Upon approval, the CAGC issues a Prov testing voucher that lets you schedule your trade exam.

Since the January 2026 transition from MASC, all new applications go through the CAGC portal. If you're working with older instructions that reference MASC forms, discard them — the CAGC has its own application process.

The Supporting Documents Checklist

Supporting documents vary by track. Prepare the items that apply to your state contractor or CAGC application:

• SLED criminal background check — required for the RBC track; order it through sled.sc.gov and check SLED's current fee and turnaround
• Credit report — where required by the board, dated within 30 days of submission
• Notarized experience affidavits — signed by each supervising contractor and notarized. Non-notarized affidavits are rejected without exception
• W-2 or 1099 tax records — matching the employment periods documented in your affidavits (RBC track)
• PSI exam pass sheets — official pass sheets from PSI (CLB track after passing; RBC track after eligibility and testing)
• Entity registration — Secretary of State filing confirmation for the applicable business-license application
• Insurance certificates — general liability policy and workers' comp where applicable (including the 4+ worker statutory trigger)
• Surety bond — if required for your track and group level

Pull the credit report last. It has the shortest validity window, and an expired report forces you to re-pull and potentially delay your submission.
