What Is Form E and Who Needs It?
Form E is the Delaware Department of Education's official document for verifying teaching experience earned outside of Delaware public or charter schools. If you are applying for a Delaware teaching license through reciprocity and need to document your years of classroom experience from another state, Form E is how that experience enters your DEEDS file.
Out-of-state experience matters because it determines which license tier you receive. Fewer than four years of verified experience under a valid out-of-state credential earns an Initial License by reciprocity. Four or more years earns a Continuing License by reciprocity.
How to Complete Form E
The form has two sections. The top section is filled out by you, the applicant. You enter your personal details, the school district where you worked, the dates of employment, and the subject areas you taught.
The bottom section must be completed and signed by a superintendent, personnel director, or authorized HR officer from your former employing school district. This section requires them to verify your dates of service, confirm that you held a valid teaching credential during that employment, and attest that you received at least two satisfactory summative evaluations.
The most recent revision of Form E is dated April 13, 2026. Use the current version — the DDOE may reject outdated forms.
The Submission Rule That Catches Everyone
Here is where most applications stall. The completed Form E must be sent directly from the former school district to the DDOE. You cannot submit it yourself. The DDOE rejects any Form E that arrives from an applicant's personal email, is uploaded by the applicant to DEEDS, or is mailed from a non-district address.
Acceptable delivery methods:
- Email from an official district domain to [email protected]
- U.S. mail from the district's official administrative address on district letterhead
If your former district is slow to respond, the most effective approach is to email the HR office directly with a pre-filled Form E (top section complete) and a cover note explaining that DDOE requires them to submit it. Include the DDOE's email address so they can send it in one step.

Common Rejection Reasons
Applicant-submitted forms. The single most frequent rejection. Even if your former superintendent signed it, DDOE will reject it if the email sender or return address is not the district itself.
Missing evaluation attestation. The form requires confirmation of at least two satisfactory summative evaluations. If your former employer leaves that section blank or writes "N/A," the DDOE flags a deficiency on your DEEDS dashboard.
Expired or invalid credential. Form E verifies experience earned under a valid teaching credential. If your out-of-state license had lapsed during part of your employment, those years do not count toward your Delaware license tier determination.
Wrong form. Form E is specifically for classroom teaching experience. Non-teaching professional experience (industry experience for CTE pathways, for example) requires Form E/NT. Clinical counseling hours require Form C.
Timeline Expectations
DDOE's processing window runs two to six weeks depending on the time of year, with peak delays between June and September. If your Form E has not been received within three weeks of your former district sending it, check your DEEDS dashboard for a deficiency flag and contact the DDOE Educator Support Team.
